Payment and
Insurance
Payment is expected when services are provided. For the
convenience of our patients, the office accepts checks and major
credit cards and will provide the forms necessary to file insurance
claims.
The office
participates with many major health care plans and will file claim
forms for these plans. The co-payment is required at the time of
the visit. If your insurance company requires a referral, please
make arrangements prior to your visit.
Medicare
For Medicare patients, the practice accepts assignment. This means the
office will accept the amount Medicare allows for a procedure or
office visit. The patient is responsible for the Medicare
deductible and 20 percent of the Medicare allowable. This amount may
be covered by your supplemental insurance plan. Please direct
