In our continuing efforts to use technology to improve our patient experience and communication, we have partnered with EyeMD to offer you an online “Clinical Summary” of your visits. This “Clinical Summary” does not contain your glasses, contact lens, or medical prescriptions. You will need to call our office to get a copy or your pharmacy to refill your prescriptions.
Register for the Patient Portal
Click here to take you to a completely secure website where you can register a username and create a password so you can login and see a brief summary of your visits since October 2013. You will need your full name as it was entered in our system, your Social Security Number (if it was given to us on your New patient forms) or Insurance ID number, and your date of birth. You will also require a default password which is available at 913.341.3100. We will confirm your identity and give you the password.
There is a new section available on the Patient Portal to send direct messages and questions to the doctor. We will strive to answer these within 24-48 hours. However, the best way to reach us and receive the quickest response will be to call us directly at 913-341-3100. We urge you to call us or 911 for the most urgent matters.
